Making Communities Healthier

Lifepoint Health built the Healthy Person Program to improve the health of the communities they serve. See how they enabled earlier diagnosis and drove downstream volume through enterprise-wide early detection.

180%

increase in patients diagnosed with early-stage cancer

>50%

reduction in time-to-treatment

80-95%

patient return rate at the top 5 hospitals

THE CHALLENGE

Standardizing early detection and care across the system

Lifepoint sought to reduce the risk of lung cancer in its communities by improving screening and tracking incidental lung nodules -- across 9 different EHRs and for over 2.4M annual radiology exams.

Key Objectives

Overcoming the complexity of disparate information systems across dozens of hospitals.
Addressing the national gap where most lung nodules are found incidentally yet only 30% receive appropriate follow-up
Transitioning from manual tracking methods to a more automated system.

THE SOLUTION

An enterprise tracking and care management program

Lifepoint launched the Healthy Lung Program to identify and longitudinally track patients with incidental abnormal lung findings. After early success, they expanded the program to 80 hospitals.

Key Enablers

Expansion into the Health Person Program to track lung pancreas thyroid AAA and breast abnormalities.
Deployment of Eon Care Management navigators expanding capacity without requiring on-site resources.
Implementation of an enterprise data cloud capable of translating across different EHRs.

THE RESULT

Improving cancer outcomes and growing service lines

Lifepoint cut time to treatment in half, supported stage shift (56% early stage diagnosis vs 20% prior), and generated over 100,000 needed follow-up exams and procedures from 2017 to 2022.

“Not only are we taking great care of our patients, but we're also seeing meaningful growth across our service lines.”
